奥氮平联合多奈哌齐治疗老年痴呆精神行为障碍的临床疗效分析

| 摘要 |
【摘要】 探讨奥氮平联合多奈哌齐治疗改善老年痴呆精神行为障碍的临床疗效。 方法 选取2013年1月-2015年1月的老年痴呆伴发精神行为障碍患者120例，按就诊奇偶顺序随机分为观察组和对照组，每组60例。观察组患者给予奥氮平联合多奈哌齐治疗，对照组给予奥氮平治疗，治疗时间为8周。采用阿尔茨海默病行为病理评定量表(BEHAVE-AD)、韦氏成人智力测试（WAIS）、临床记忆量表和药物副反应量表（TESS）比较两种药物的临床疗效及不良反应并进行统计学分析。结果 两组患者治疗后2、4、8周的BEHAVE-AD均较治疗前显著降低（P<0.05）治疗8周后，观察组的总有效率为94.25%，对照组的总有效率为92.16%，差异无统计学意义（P>0.05）; 韦氏成人智力测试（WAIS）、临床记忆量表评分观察组明显高于对照组，差异有统计学意义（P<0.01）;奥氮平使用剂量观察组明显高于对照组，差异有统计学意义（P<0.01）。观察组较对照组不良反应发生率低，差异有统计学意义（P<0.05）。结论 奥氮平联合多奈哌齐治疗老年痴呆精神行为障碍疗效较单用奥氮平认知功能改善好，不良反应发生率低，治疗安全性高，奥氮平使用剂量低，值得临床推广。

| Abstract |
【Abstract】 Objective Tinvestigate the efficacy and safety of Olanzapine joint Donepezil in the treatment of Alzheimer’s behavioral and psychological symptoms. Methods 120 patients with behavioral and psychological symptoms of dementia patients were randomly divided into study group 60 cases and the control group 60 cases, patients in shudy group were treated with Olanzapine joint Donepezil , patients in control group were treated with Olanzapine . The treatment time was 8 weeks. Before treatment and after 2、4、8 weeks of treatment on patients with the behavioral pathology in Alzheimer’s Disease Scale(BEHAVE-AD) 、Wechsler adult intelligence scale（WAIS）, clinical memory scale(CMS), treatment emergent symptoms scale（TESS） scale score. Statistical analysis was conducted. Results 2,4,8weekks after treatment, BEHAVE-AD scale scores of the two groups were significantly lower than those before treatment, the differences were statistically significant (P<0.05). During 8 weeks treatment ,the effective rate in observation group was 94.25% and in control group was 92.16% ,there was no significant difference in the efficacy between the two groups . The efficacy and side effects in observation group was lower than that of control group, there was significant difference in the efficacy between the two groups (P<0.05) . Wechsler adult intelligence scale（WAIS）and clinical memory scale(CMS) in observation group was higher than that of control group , there was significant difference in the efficacy between the two groups (P<0.05) . The dosage of Olanzapine in observation group was lower than that of control group(P<0.05). Conclusion Olanzapine joint Donepezil in the treatment for improving cognition of Alzheimer’s behavioral and psychological symptoms has a better clinical efficacy. It cause less adverse reaction and has better safety .The dosage of Olanzapine was lower .Therefore , Olanzapine joint Donepezil deserves further clinical applications .
