精神分裂症伴抑郁症患者血清iNOS、TrkB、SHBG表达及其与PANSS评分的相关性

【摘要】 目的 研究精神分裂症伴抑郁症患者血清诱导型一氧化氮合酶（iNOS）、酪氨酸激酶受体B（TrkB）、激素结合蛋白（SHBG）表达及其与阳性与阴性症状量表（PANSS）评分的相关性。方法 以2023年9月~2024年9月襄阳市安定医院收治的126例精神分裂症伴抑郁症患者为病例组，另取同期未伴抑郁症精神分裂症患者126例作为对照组A及126名无精神疾病健康体检者作为对照组B。结果 病例组PANSS评分各项及总分、血清iNOS、SHBG表达水平均高于对照组A、对照组B，其中对照组A均高于对照组B，血清TrkB表达水平则均低于对照组A、对照组B，其中对照组A均低于对照组B（P<0.05）。精神分裂症伴抑郁症患者血清iNOS、SHBG表达水平与PANSS评分呈正相关（r=0.796、0.611，P<0.05），精神分裂症伴抑郁症患者血清TrkB表达水平则与PANSS评分呈负相关（r=-0.652，P<0.05）。将对照组A患者作为阴性对照，病例组患者作为阳性病例，ROC曲线分析结果显示，血清iNOS、TrkB、SHBG表达水平联合对精神分裂症伴抑郁症患者的诊断曲线下面积（AUC）高于三者单独检测（P<0.05），其敏感度和特异度分别为92.86%和83.33%。结论 精神分裂症伴抑郁症患者血清iNOS、TrkB、SHBG表达水平与PANSS评分均具有高度的相关性，且三者联合对精神分裂症伴抑郁症患者的诊断价值较高。

| Abstract |
[Abstract] Objective To study the expression of serum inducible nitric oxide synthase (iNOS), tyrosine kinase receptor B(TrkB) and hormone binding protein (SHBG) in schizophrenic patients with depression and their correlation with the scores of Positive and Negative Symptom Scale (PANSS). Methods From September 2023 to September 2024, 126 schizophrenic patients with depression admitted to Anding Hospital of Xiangyang City were taken as the case group, and 126 schizophrenic patients without depression in the same period were taken as the control group A and 126 healthy people without mental illness as the control group B. Results The all items and total scores of PANSS, expression levels of serum iNOS and SHBG in the case group were higher than those in the control group A and the control group B. Among them, the control group A was higher than the control group B, while the expression levels of serum TrkB were lower than those in the control group A and the control group B. Among them, the control group A was lower than the control group B (P<0.05). The expression levels of serum iNOS and SHBG in schizophrenic patients with depression were positively correlated with score of PANSS (r=0.796, 0.611, P<0.05), while the expression level of serum TrkB in schizophrenic patients with depression was negatively correlated with score of PANSS (r=-0.652, P<0.05). Patients in the control group A were taken as negative control, while patients in the case group were taken as positive cases. The analysis of ROC curve showed that the combined expression levels of serum iNOS, TrkB and SHBG were higher than the area under the diagnostic curve (AUC) of schizophrenic patients with depression (P<0.05), and their sensitivity and specificity were 92.86% and 83.33% respectively. Conclusion The expression levels of serum iNOS, TrkB and SHBG in schizophrenic patients with depression were highly correlated with score of PANSS, and the combination of them had high diagnostic value for schizophrenic patients with depression.
