摘要	摘要：目的：分析不同频率的重复经颅磁刺激对于老年痴呆患者的认知功能与精神行为症状的影响。方法：采用随机列表法选取自2014年10月到2016年12月在我院接受治疗的伴有精神行为症状的轻中度老年痴呆患者180例，随机分为5Hz组、15Hz组和对照组，每组各60例，三组患者均予神经内科老年痴呆的基础疗法，不给予改善认知功能的药物与认知功能康复训练，同时三组患者分别予5Hz、15Hz r TMS与伪刺激。治疗前后分别行简易精神状态检查量表评分（MMSE）、蒙特利尔认知评分（ MoCA）、病理行为量表评分(BEHAVE-AD)、日常生活能力量表评分(ADL)、神经精神量表评分(NPI)、Boston命名测验评分(BNT)以及数字广度测验评分(DST)，比较三组的疗效。结果：治疗后，5Hz组与15Hz组的MMSE评分、MoCA评分、DST评分、BNT评分与总有效率均比对照组显著升高，ADL评分与NPI评分比对照组显著降低（P<0. 05）；治疗后，5Hz组的MMSE评分、MoCA评分、DST评分、BNT评分与总有效率均比15Hz组的显著升高， NPI评分与均比对照组显著降低（P<0. 05），两组的ADL评分没有统计学方面的差异（P>0.05）。结论：重复经颅磁刺激能够有效地改善老年痴呆患者的认知功能与精神行为症状，且在高频重复经颅磁刺激中，5Hz的频率的作用更强，对患者的安全性较高，可应用于临床。
Abstract	Abstract: Objective: To analysis the effect of repetitive transcranial magnetic stimulation （ r TMS ） with different frequency on cognitive function and behavioral and psychological symptoms in patients with senile dementia. Methods 180 patients with behavioral and psychological symptoms were randomly selected and divided into 5Hz group , 15Hz group and control group from October 2014 to June 2016 in our hospital. Besides standard basic treatment for Alzheimer’s disease，the three groups were treated with 5Hz r TMS , 15Hz r TMS and sham r TMS, respectively. The behavioral pathology in BEHAVE-AD, MMSE, MoCA, ADL, NPI, BNT, DST were assessed before and after the treatment. The effect of three groups was compared after treatment. Results The scores of MMSE, MoCA, BNT, DST and total curative effect in 5Hz group and 15Hz group were significantly higher than those in control group（P<0. 05）after treatment. The scores of NPI and ADL in 5Hz group and 15Hz group were significantly lower than those in control group（P<0. 05）after treatment. The scores of MMSE, MoCA, BNT, DST and total curative effect in 5Hz group were significantly higher than those in 15Hz group（P<0. 05）after treatment. The scores of NPI in 5Hz group were significantly lower than those in 15Hz group（P<0. 05）after treatment. The scores of ADL in 5Hz group and 15Hz groups had no significant difference（P>0.05）. Conclusion r TMS can effectively improve the Alzheimer’s behavioral and psychological symptoms and cognitive function with high safety. Particularly good effect can be achieved with 5Hz r TMS . It can be used in clinic.
