抗精神药物联合认知行为疗法对慢性精神分裂症阴性症状的疗效观察

摘要 观察抗精神药物联合认知行为疗法对慢性精神分裂症阴性症状的疗效。方法 筛选我院2012年3月-2012年9月住院>2年并确诊为慢性精神分裂症阴性症状的患者80例，随机分为对照组和观察组（各40例），两组均维持原抗精神药物治疗，观察组结合认知行为疗法治疗，均治疗8周，比较两组疗效，治疗前后认知功能成套测验系统（MCCB）评分、简明精神病量表（BPRS）评分、阴性症状量表（SANS）评分和临床疗效总评量表（CGI-GI）评分变化。结果 观察组治疗总有效率92.5%，对照组75%，差异具有统计学意义（P<0.05）；治疗后两组MCCB各项目评分均显著降低（P<0.05），BPRS和SANS评分显著降低（P<0.05），CGI-GI评分显著升高（P<0.05），且观察组降低或升高幅度大于对照组，差异具有统计学意义（P<0.05）。结论 抗精神药物治疗基础上联合认知行为治疗，可显著改善慢性精神分裂症患者阴性症状，增强患者认知功能，疗效确切，值得推荐。

| Abstract |
Abstract Objective To observe the effect of antipsychotic drugs combined with cognitive behavioral therapy on negative symptoms of chronic schizophrenia. Methods A total of 80 patients with negative symptoms of chronic schizophrenia who were hospitalized in our hospital for longer than 2 years between March 2012 and September 2012 were randomly divided into the control group and the observation group, 40 cases in each group. Both groups maintained the original antipsychotic treatment, and the observation group were additionally given cognitive behavioral therapy. Both groups were treated for 8 weeks, and the efficacy, changes of MATRICS Consensus Cognitive Battery (MCCB) score, the Brief Psychiatric Rating Scale (BPRS) score, scale for the assessment of negative symptoms (SANS) score and the Clinical Global Impression Scale (CGI-GI) score before and after treatment were compared between the two groups. Results The total effective rate in the observation group was 92.5% while in the control group was 75% (P<0.05). After treatment, the scores of MCCB, BPRS and SANS were decreased significantly while scores of CGI-GI were increased significantly (P<0.05), and changes were more significantly in the observation group than the control group (P<0.05). Conclusion The combination of antipsychotic drugs and cognitive behavioral therapy can significantly relieve the negative symptoms of patients with chronic schizophrenia and enhance the cognitive function of patients.
