家庭医生签约服务模式在老年慢性病社区护理中的应用效果观察

摘要	目的剖析家庭医生签约服务模式用于老年慢性病社区护理的作用。方法 2024年全年本社区医院接诊的老年慢性病病人中随机抽取120人，并用电脑随机双盲法将之均分成2组。试验组采取社区护理和家庭医生签约服务模式，对照组只接受社区护理。对比ESCA评分等指标。结果关于ESCA评分；干预后，试验组（132.59±9.15）分，对照组（102.59±8.41）分，差异显著（P＜0.05）。健康知识认知度：试验组（91.25±3.76）分，对照组（64.29±5.28）分，差异显著（P＜0.05）。sf-36评分：干预后，试验组（89.72±3.13）分，对照组（83.61±4.76）分，差异显著（P＜0.05）。结论老年慢性病社区护理期间运用家庭医生签约服务模式，利于病人自护能力、健康知识认知度与生活质量的改善。
Abstract	Objective To analyze the role of the family doctor contract service model in community nursing for elderly chronic diseases. Methods 120 elderly chronic disease patients admitted to our community hospital throughout 2024 were randomly selected and divided into two groups using a computer randomized double-blind method. The experimental group adopted a community nursing and family doctor contract service model, while the control group only received community nursing. Compare ESCA scores and other indicators. Results Regarding ESCA score; After intervention, the experimental group scored (132.59 ± 9.15) points, while the control group scored (102.59 ± 8.41) points, with a significant difference (P<0.05). Health knowledge awareness: The experimental group scored (91.25 ± 3.76) points, while the control group scored (64.29 ± 5.28) points, with a significant difference (P<0.05). SF-36 score: After intervention, the experimental group scored (89.72 ± 3.13) points, while the control group scored (83.61±4.76) points, with a significant difference (P<0.05). Conclusion The use of family doctor contract service model during community nursing for elderly chronic diseases is beneficial for improving patients' self-care ability, health knowledge awareness, and quality of life.
