摘要	评估奥氮平、喹硫平2种非典型抗精神病药对阿尔茨海默病(Alzheimers disease,AD)患者精神行为症状的疗效、心电图校正QT间期(corrected QT interval,QTc)及PR间期的影响。方法观察144例AD患者分别为奥氮平组(n = 48) 、喹硫平组(n = 34) 、对照组(n = 62)，药物治疗4周后，观测简明精神病评定量表(Brief Psychiatric Rating Scale,BPRS)、简易智力状态检查量表(Mini-mental State Examination,MMSE)评分及心电图QTc间期、PR间期变化并进行比较。结果与治疗前相比，奥氮平组与喹硫平组BPRS评分显著降低（均P<0.05），奥氮平组治疗后MMSE评分显著降低(P<0.05)；治疗后非典型抗精神病药组Q T c间期延长发生率显著高于对照组（P=0.007）；QTc间期较对照组延长（F=3.102，P=0.048）。三组治疗前后PR间期均无差异。结论奥氮平、喹硫平对于AD患者的BPRS症状有疗效，对于认知功能短期内影响较小。奥氮平、喹硫平等有延长AD患者QTc间期的风险。
Abstract	The evaluation of olanzapine and quetiapine 2 kinds of atypical antipsychotics on efficacy, behavioral and psychiatric symptoms in patients with Alzheimers disease were corrected QT interval (corrected QT interval，QTc) and PR interval. Methods 144 patients with AD were treated with olanzapine (n = 48), quetiapine (n = 34), and control (n = 62). The BPRS, MMSE and ECG QTc interval and PR interval were compared before and after 4 weeks treatment. Results Compared with before treatment, the BPRS scores of olanzapine group and quetiapine group decreased significantly (P < 0.05), and the scores of MMSE in olanzapine group decreased significantly after treatment (P < 0.05). After treatment, the incidence of QTc interval prolongation in atypical antipsychotics group was significantly higher than that in control group (P=0.007), and the QTc interval was longer than that in control group (F=3.102, P=0.048). There was no difference in PR interval between the three groups before and after treatment. Conclusion Olanzapine and quetiapine have efficacy in the treatment of BPRS symptoms in AD patients and have less impact on cognitive function in the short term. Olanzapine and quetiapine have an increased risk of QTc interval in patients with AD.
