摘要
无抽搐电休克治疗(MECT)是治疗多种精神疾病的有效安全的治疗方法。但MECT存在认知功能障碍等不良反应,且老年精神障碍患者往往合并较多躯体问题,MECT的使用往往受到限制。目前关于老年精神患者应用MECT治疗的相关研究较少。本文主要探讨常见老年精神疾病应用MECT疗效的研究和风险评估。通过治疗前评估,治疗中及治疗后的严密监测,可有效减少MECT不良反应。综合来讲,MECT起效快、安全性高、不良反应少,对于药物治疗无效或者不能耐受药物副作用、出现严重的躁狂发作或抑郁症、难治性精神分裂症等重性精神疾病的老年患者,MECT应作为一线治疗方案。
Abstract
Modified electroconvulsive therapy(MECT) is well established as a safe and effective treatment for several mental disorders. However, MECT has some adverse reactions, and geriatric patients may have more somatic problems, thus the use of MECT in the elderly is often limited. This article mainly discusses the research and risk assessment of the use of MECT in geriatric patients who suffer from mental disorders. The adverse reactions can be effectively reduced by pre-treatment evaluation, intensive monitoring the vital signs during treatment and after treatment. Taken together, the application of MECT should be as a first-line treatment which is safer, more rapid onset of action, fewer side effects in elderly patients suffering from severe psychiatric disorders.
