摘要
探究老年慢性心力衰竭(CHF)伴抑郁症患者应用氟西汀治疗的临床效果。方法 掷币法将97例老年CHF伴抑郁症患者分为2组,常规组48例予常规心理干预和抗心衰治疗,试验组49例加用氟西汀,均治疗3个月,对比两组治疗前后心功能、抑郁情况、免疫细胞因子水平及安全性。结果 试验组1月、2月及3月末汉密尔顿抑郁量表(HAMD)评分均低于常规组,6min平地步行距离(6WMT)、心脏彩超对心排量(CO)、左心射血分数(LVEF)、血清5-羟色胺(5-HT)、CD3+、CD4+及CD8+水平均高于常规组(P<0.05),QTc 3月末水平及不良反应两组无明显差异(P>0.05)。结论 氟西汀应用于老年CHF抑郁症患者,可提高其相关神经递质及免疫细胞因子水平,改善其抑郁及心功能,安全有效。
Abstract
To investigate the clinical effects of fluoxetine in elderly patients with chronic heart failure (CHF) and depression. Methods 97 elderly patients with CHF and depression were divided into two groups. The conventional group received 48 cases of routine psychological intervention and anti-heart failure treatment. The experimental group received 49 cases of fluoxetine for 3 months. Cardiac function, depression, immune cytokine levels and safety were comperad. Results The Hamilton Depression Scale (HAMD) scores of the experimental group after 1、2 and 3 months were lower than the conventional group, 6 min walking distance (6 WMT), cardiac color Doppler (CO), left cardiac ejection fraction (LVEF). Serum serotonin (5-HT), CD3+, CD4+ and CD8+ levels were higher than the conventional group (P<0.05), and there was no significant difference on the QTc levels and adverse reactions (P>0.05). Conclusion Fluoxetine can improve the level of related neurotransmitters、 immune cytokines、depression and cardiac function in elderly patients with CHF depression. safely and effectively.
