目的:观察脑梗死MRI表现与患者认知功能障碍、抑郁症状的相关性。方法:将2018年1月~2018年12月92例在我院接受治疗的脑梗死患者纳入研究对象,根据梗死部位的不同分为丘脑组、内囊-基底节组、小脑组及脑干组,将同期在我院健康体检的90例正常者作为对照组,对所有纳入对象精神状态评分(MMSE)、Hachinsi缺血指数(HIS)以及抑郁评分(HAMD)进行评价,予以统计学分析。结果:与对照组相比,丘脑组、内囊-基底节组、小脑组、脑干组MMSE评分均较低,内囊-基底节组HAMD评分较对照组高(P<0.05),有统计学意义;HAMD评分比较左侧梗死组显著低于对照组,左侧组MMSE评分低于右侧组,抑郁组MMSE评分显著低于非抑郁组(P<0.05),有统计学意义。结论:丘脑、小脑以及脑干等部位梗死会引起血管性痴呆,病灶位于左侧及抑郁患者更易发生痴呆,左侧梗死及内囊-基底节病变容易发生抑郁。

| Abstract |
Objective: To observe the correlation between MRI findings of cerebral infarction and cognitive dysfunction and depressive symptoms. Methods: 92 patients with cerebral infarction treated in our hospital from January 2018 to December 2018 were included in the study. They were divided into thalamus group, internal capsule-basal ganglia group, cerebellum group and brainstem group according to the location of infarction. Ninety normal persons who underwent physical examination in our hospital during the same period were taken as control group. The mental state score (MMSE), Hachinsi ischemic index (HIS) and depression score of all the subjects included were evaluated. The score (HAMD) was evaluated and analyzed statistically. Results: Compared with the control group, the MMSE scores of thalamus group, internal capsule-basal ganglia group, cerebellum group and brainstem group were lower, and the HAMD scores of internal capsule-basal ganglia group were higher than those of control group (P < 0.05), with statistical significance; the HAMD scores of left infarction group were significantly lower than those of control group, the MMSE scores of left infarction group were lower than those of right group, and the MMSE scores of depression group were significantly lower than those of non-depression group (P < 0.05). Significance. Conclusion: Cerebral infarction in thalamus, cerebellum and brainstem can cause vascular dementia. Patients with left cerebral infarction and depression are more likely to develop dementia, while those with left cerebral infarction and internal capsule-basal ganglia are more likely to develop depression.
