抗抑郁药物联合阿片类药物治疗晚期癌痛伴抑郁症的疗效观察

| 摘要 |
探讨抗抑郁药物联合阿片类药物治疗晚期癌痛伴抑郁症的疗效。方法 依据随机对照原则将2017年1月~2019年1月我院进行治疗的80例晚期癌痛伴抑郁症患者分为每组40例，对照组患者接受阿片类药物治疗，观察组患者在此基础上加用抗抑郁药物治疗。比较两组疗效及安全性。结果 观察组患者治疗第7d、第14d、第28d阿片类药物使用剂量均低于对照组，差异显著（P＜0.05）；观察组患者治疗第28d癌痛频率及数字分级法（NRS）评分均低于对照组，差异显著（P＜0.05）；观察组患者治疗后汉密尔顿抑郁量表（HAMD）评分低于对照组，生存质量评分高于对照组，差异显著（P＜0.05）；两组患者治疗期间不良反应总发生率并无统计学差异（P＞0.05）。结论 抗抑郁药物联合阿片类药物治疗晚期癌痛伴抑郁症疗效显著，安全性良好，值得临床推荐。

| Abstract |
Objective To explore the curative effect in the terminal cancer pain with depressive disorder patients by antidepressant and opioids. Methods 80 cases of terminal cancer pain with depressive disorder patients from 2017/01 to 2019/01 were randomly divided into observation group(40 cases) and control group(40 cases). The control group were treated by opioids, the observation group were treated by antidepressant and opioids. The curative effect and safety were observed. Results The dosage of opioids for the observation group in 7d, 14d, 28d after treatment were lower than the control group(P＜0.05); The score of NRS and HAMD for the observation group in 28d after treatment were lower than the control group(P＜0.05); The quality of life score for the observation group in 28d after treatment were higher than the control group(P＜0.05); There were no significant difference on the untoward effect rate between the two groups(P＞0.05). Conclusion It has prominent curative effect in the terminal cancer pain with depressive disorder patients by antidepressant and opioids. It has good safety. It worth of clinical recommendations.
