摘要:探讨产后抑郁患者下丘脑-垂体-肾上腺(HPA)轴、甲状腺功能及性激素水平的变化及其意义。方法 选取产后6~7周经我院精神科门诊检测评估为产后抑郁的产妇100例作为抑郁组、100例同期产后未发生抑郁的产妇作为对照组;检测对比两组的HPA激素、甲状腺功能激素、性激素水平;并分析抑郁组患者各项激素水平与爱丁堡产后抑郁量表(EPDS)评分的关系。结果 抑郁组患者的CHR、ACTH水平高于对照组(P<0.05),抑郁组患者的CORT水平低于对照组(P<0.05);抑郁组患者的TSH水平低于对照组(P<0.05),抑郁组患者的TG-Ab、TPO-Ab、FT3、FT4水平与对照组比较,差异无统计学意义(P>0.05);抑郁组患者的E2水平低于对照组(P<0.05),抑郁组患者的PRL、P值高于对照组(P<0.05);抑郁组患者的E2、TSH、CORT水平与EPDS评分呈显著的负相关关系(P<0.05),抑郁组患者的PRL、CHR、ACTH测定值与EPDS评分呈显著的正相关关系(P<0.05)。结论 产后抑郁患者受自身激素水平的变化较为明显,调节产后激素水平有利于减轻产后抑郁的病情和治疗。

[Abstract]Objective: To investigate the changes and significance of hypothalamus-pituitary-adrenal (HPA) axis, thyroid function and sex hormone levels in postpartum depression patients. Methods: 100 pregnant women with postpartum depression assessed by our psychiatric outpatient clinic 6 to 7 weeks after delivery were selected as the depression group, and 100 pregnant women who did not experience depression during the same period were selected as the control group; HPA hormones, thyroid function hormones, Sex hormone levels; and analyzed the relationship between the hormone levels of patients in the depression group and the Edinburgh Postpartum Depression Scale (EPDS) score. Results: Patients in the depression group had higher levels of CHR and ACTH (P <0.05), patients in the depression group had lower levels of CORT (P <0.05); patients in the depression group had lower TSH levels than the control group (P <0.05), The levels of TG-Ab, TPO-Ab, FT3, and FT4 in patients in the depression group were not significantly different from those in the control group (P> 0.05); E2 levels in patients in the depression group were lower than those in the control group (P <0.05). The PRL and P values of the patients were higher than those of the control group (P <0.05). The levels of E2, TSH, and CORT in the depression group were significantly negatively correlated with the EPDS scores (P <0.05). The PRL, CHR, and ACTH of the depression group were significantly lower. There was a significant positive correlation between the measured value and EPDS score (P <0.05). Conclusion: Postpartum depression patients are significantly affected by changes in their own hormone levels. Adjusting the postpartum hormone level is beneficial to alleviate the condition and treatment of postpartum depression.
