【摘要】 探讨重度抑郁症患者经氨磺必利联合帕罗西汀治疗效果,分析对突触相关miRNAs表达影响。方法 选取2018年4月-2019年10月期间我院确诊并接受治疗124例重度抑郁症患者,采用数字奇偶法将入选者分为观察组(奇数)与对照组(偶数),各62例。全部患者均接受常规治疗与帕罗西汀治疗,在此基础上为观察组患者联合氨磺必利治疗,两组均治疗3个月检测患者突触相关miRNAs(miR-124、miR-132、miR-134、miR-138)表达量、阴性症状评定量表(SANS)评分及不良反应。结果 治疗3个月后,观察组miR-124、miR-134及miR-138表达量均低于对照组,miR-132表达量高于对照组(P<0.05);观察组低于对照组(P<0.05);治疗过程中,两组不良反应发生率相近(P>0.05)。结论 重度抑郁症患者经氨磺必利联合帕罗西汀治疗获益明显,患者突触相关miRNAs表达明显改善,有利于改善阴性症状,且联合用药不会增加不良反应发生率,安全性高,临床应用价值高。

[Abstract] Objective To explore the therapeutic effect of amisulpride and paroxetine in patients with major depression, and to analyze the effect on synapse-related miRNAs expression. Methods From April 2018 to October 2019, 124 patients with severe depression diagnosed who treated in our hospital were divided into observation group (odd number) and control group (even number) according to digital parity method, 62 cases each. All patients received conventional treatment and paroxetine treatment. Patients in the observation group were treated with amisulpride on this basis. Both groups were treated for 3 months, detected the expression level of synapse-related miRNAs (miR-124, miR-132, miR-134, miR-138), negative symptom rating scale (SANS) scores and adverse reactions. Results After 3 months of treatment, the expression levels of miR-124, miR-134 and miR-138 in the observation group were lower than those in the control group, and the expression levels of miR-132 were higher than the control group (P<0.05); the observation group was lower than the control group (P<0.05); during the treatment, the incidence of adverse reactions was similar between the two groups (P>0.05). Conclusion Patients with severe depression benefit significantly from the treatment of amisulpride and paroxetine. The expression of synaptic-related miRNAs in patients was significantly improved, which was conducive to the improvement of negative symptoms. Combined medication will not increase the incidence of adverse reactions, it has high safety and value of clinical application.
