【摘要】目的：探讨维持性血液透析自体动静脉内瘘(AAVF)术后老年患者应用正念减压疗法（MB-SR）对血管功能、负性情绪及应对方式的影响。方法：选取123例维持性血液透析AAVF术后老年患者作为研究对象，采用随机数字表法分组。两组患者均给予内瘘局部热敷、握拳与放松训练、远红外线照射及常规护理干预，对照组61例增加知识教育模式，观察组62例增加MB-SR疗法。观察两组患者动静脉内瘘血管功能、负性情绪、疾病应对方式及动静脉内瘘相关并发症。结果：观察组患者动静脉内瘘血管内径、血管壁厚度、血流量均大于对照组（P<0.05）；观察组汉密尔顿焦虑量表（HAMA）、汉密尔顿抑郁量表（HAMD）评分均小于对照组（P<0.05）；观察组患者屈服评分小于对照组，面对、回避评分均大于对照组（P<0.05）；观察组患者并发症发生率明显小于对照组（P<0.05）。结论：MB-SR联合常规护理能够增强维持性血液透析AAVF术后老年患者血管功能，改善心理状态及应对疾病的方式。

| Abstract |
[Abstract] Objective: To investigate the effects of mindfulness-based decompression therapy (MB-SR) on vascular function, negative emotions and coping styles in elderly patients after maintenance hemodialysis autologous arteriovenous fistula (AAVF). Methods: A total of 123 elderly patients with AAVF after maintenance hemodialysis were selected as the research object, and grouped by random number table. Two groups of patients were given local fistula hot compress, clench and relaxation training, far infrared radiation and routine nursing intervention, 61 cases in the control group increased knowledge education mode, 62 cases in the observation group increased MB-SR therapy. Vascular function, negative emotion, disease coping style and complications related to arteriovenous fistula in 2 groups were observed. RESULTS: The internal diameter, wall thickness and blood flow of arteriovenous internal fistula in observation group were higher than those in control group (P<0.05). Hamilton Anxiety Scale (HAMA) and Hamilton Depression Scale (HAMD) scores in the observation group were lower than those in the control group (P<0.05). The yield score of observation group was lower than control group, and the face and avoidance scores of observation group were higher than control group (P<0.05). The incidence of complications in observation group was significantly lower than that in control group (P<0.05). Conclusion: MB-SR combined with routine nursing can enhance vascular function, improve mental state and the way of coping with disease in elderly patients after AAVF maintenance hemodialysis.
