【摘要】探讨学龄前期龋齿患儿牙科焦虑症现状及与口腔健康相关生活质量的关系。方法 本研究对象为我院2019年1月~2020年12月104例学龄前期龋齿患儿,采用改良牙科焦虑问卷表(MDAS)进行牙科焦虑症筛查,并设为焦虑组以及非焦虑组,比较两组患儿临床资料;采用中文版学龄前儿童口腔健康影响量表(ECOHIS)评估口腔健康相关生活质量,并分析牙科焦虑症与口腔健康相关生活质量的关系。结果 104例学龄前期龋齿患儿检出牙科焦虑症37例,发生率35.57%,焦虑组以及非焦虑组年龄、性别、父母最高学历、居住地差异无统计学意义(P>0.05);两组牙科不良就诊体验史、龋齿数量差异具有统计学意义(P<0.05);焦虑组ECOHIS量表中口腔功能、口腔症状、患儿心理、社会关系、父母内疚感评分显著高于非焦虑组,(P<0.05),两组家庭经济影响评分差异无统计学意义(P>0.05);pearson相关分析显示,MDAS评分与ECOHIS量表中口腔功能、口腔症状、患儿心理、社会关系、父母内疚感评分呈正相关(P<0.05),与家庭经济影响评分无相关性(P>0.05)。结论 学龄前期龋齿患儿牙科焦虑症检出率较高,可能与既往牙科不良就诊体验史、龋齿严重程度有关,牙科焦虑症对口腔功能、父母内疚感等口腔健康相关生活质量具有显著影响。
Abstract
[Abstract] Objective To explore the current status of dental anxiety in preschool children with dental caries and its relationship with oral health-related quality of life. Methods A total of 104 preschool children with dental caries from January 2019 to December 2020 in our hospital were selected as the study subjects. The modified dental anxiety questionnaire (MDAS) were used to screen for dental anxiety and were set as the anxiety group and the non-anxiety group. The clinical data of the two groups of children were compared. The Chinese version of the oral health impact scale for preschool children (ECOHIS) was used to assess oral health-related quality of life, and the relationship between dental anxiety and oral health-related quality of life were analyzed. Results 104 cases of preschool children with dental caries were diagnosed with 37 cases of dental anxiety, with an incidence rate of 35.57%. There was no significant difference in age, gender, parents’ highest education, and residence between the anxiety group and the non-anxiety group (P>0.05). There was a statistically significant difference in the history of poor dental visits and the number of dental caries between the two groups (P<0.05). The scores of oral function, oral symptoms, children’s psychology, social relations, and parental guilt in the ECOHIS scale of the anxiety group were significantly higher than those of the non-anxiety group (P<0.05). There was no statistically significant difference in family economic impact scores between the two groups (P>0.05). Pearson correlation analysis showed that the MDAS score was positively correlated with oral function, oral symptoms, children’s psychology, social relations, and parental guilt scores in the ECOHIS scale (P<0.05), but no correlation with family economic impact scores (P>0.05). Conclusion The detection rate of dental anxiety in preschool children with dental caries is higher, which may be related to the history of poor dental visits and the severity of dental caries. Dental anxiety has a significant impact on oral function, parental guilt and other oral health-related quality of life.
