【摘要】目的 探究经皮肾镜取石术老年患者麻醉前应用右美托咪定(DEX)的效果观察。方法 纳入本科121例经皮肾镜取石术老年患者以随机数字表法分组，对照组60例麻醉诱导前给予等量生理盐水，观察组61例麻醉诱导前给予DEX静脉滴注，对比两组患者术后认知功能、心理状态、胰岛素抵抗现象及肾功能。结果 术后3d，观察组简明智能状态量表(MMSE)评分水平高于对照组（P＜0.05）；抑郁自评量表（SDS）和焦虑自评量表（SAS）水平较对照组低（P＜0.05）；空腹血糖（FBG）、胰岛素抵抗（IR）及血清胰岛素水平（FINs）较对照组低（P＜0.05）；血清胱抑素（Cys-C）、尿α-微球蛋白水平（α1-MG）低于对照组（P＜0.05）。 结论 麻醉诱导前应用DEX在预防老年经皮肾镜取石术患者术后认知障碍、不良心理状态、胰岛素抵抗以及肾功能损伤方面效果良好。

| Abstract |
【Abstract】Objective To explore the effect of dexmedetomidine (DEX) before anesthesia in elderly patients undergoing percutaneous nephrolithotomy. Methods A total of 121 elderly patients undergoing percutaneous nephrolithotomy were divided into groups by random number table. In the control group, 60 patients were given the same amount of normal saline before anesthesia induction, and 61 patients in the observation group were given intravenous DEX before anesthesia induction. Post-cognitive function, mental state, insulin resistance and renal function. Results 3 days after the operation, the scoring level of the Concise Intelligence State Scale (MMSE) of the observation group was higher than that of the control group (P＜0.05); the level of Self-Rating Depression Scale (SDS) and Self-Rating Anxiety Scale (SAS) was lower than that of the control group ( P＜0.05); fasting blood glucose (FBG), insulin resistance (IR) and serum insulin levels (FINs) were lower than those of the control group (P＜0.05); serum cystatin (Cys-C) and urine α-microglobulin levels (Α1-MG) was lower than the control group (P<0.05). Conclusion Application of DEX before induction of anesthesia is effective in preventing postoperative cognitive impairment, poor mental state, insulin resistance and renal function damage in elderly patients undergoing percutaneous nephrolithotomy.
