【摘要】 探讨小剂量艾司氯胺酮对老年患者下肢手术后认知功能及负性情绪的影响。方法 本研究对象为我院2019年2月-2022年1月90例老年下肢手术患者，随机均分为对照组与观察组，对照组进行常规全身麻醉，观察组在对照组基础上给予小剂量艾司氯胺酮。评估两组患者术前、术后1d、5d蒙特利尔认知评估量表(Montreal Cognitive Assessment Scale，MoCA)、汉密尔顿抑郁量表（Hamilton Depression Scale，HAMD）、汉密尔顿焦虑量表（Hamilton Anxiety Scale，HAMA），比较术后舒芬太尼用量、苏醒时间以及认知功能障碍发生率。结果 两组术后1d、5d MoCA评分均显著低于术前，HAMD评分、HAMA评分显著高于术前，且观察组MoCA评分高于对照组，HAMD评分、HAMA评分低于对照组，（P<0.05），观察组术后舒芬太尼用量、苏醒时间、认知功能障碍发生率显著低于对照组，（P<0.05）。结论 小剂量艾司氯胺酮能够降低老年下肢手术后认知功能损伤，降低负性情绪，减少舒芬太尼用量，促进术后苏醒。

| Abstract |
[Abstract] Objective To investigate the effect of femoral low dose esketamine on cognitive function and negative emotion after lower extremity surgery in the elderly patients. Methods Ninety elderly patients with lower extremity surgery in our hospital from February 2019 to January 2022 were selected as the study subjects, they were randomly divided into the control group and the observation group. The control group was given routine general anesthesia, and the observation group was given low dose esketamine on the basis of the control group. The Montreal Cognitive Assessment Scale (MoCA), Hamilton Depression Scale (HAMD) and Hamilton Anxiety Scale (HAMA) were evaluated before operation, 1d and 5d after operation in the two groups. The postoperative sufentanil dosage, recovery time and incidence of cognitive dysfunction were compared. Results The MoCA scores of the two groups at 1d and 5d after the operation were significantly lower than those before the operation, and the HAMD and HAMA scores were significantly higher than those before the operation (P<0.05). The postoperative sufentanil dosage, recovery time and the incidence of cognitive dysfunction in the observation group were significantly lower than those in the control group (P<0.05). Conclusion Low dose esketamine can reduce cognitive impairment, reduce negative emotions, reduce sufentanil dosage, and promote postoperative recovery in elderly patients after lower limb surgery.
