Objective: To systematically evaluate the efficacy and safety of blonanserin for the treatment of schizophrenia in adults based on the current literature. Methods: Double-blind controlled random trials of blonanserin for the treatment of schizophrenia in adults were searched through the internet in relevant databases, such as Medlin, Pubmed, Embase, CNKI, Sinomed, and Wanfang in the range from the database founded to December 2022. Meta-analysis were performed on articles included. Results: 6 included articles showed that there was no difference between blonanserin and risperidone in improving PANSS total scores, positive scale scores and negative scale scores. Haloperidol was superior to blonanserin in improving PANSS scores. Blonanserin was less likely not even than risperidone to cause hyperprolactinemia (P < 0.001), elevating serum prolactin levels (P < 0.001), elevating alanine aminotransferase levels (P=0.004), elevating aspartate aminotransferase levels (P=0.04), or increasing body weight (P=0.005), but also than haloperidol to cause tremors (P=0.001) and akathisia (P=0.01). No significant difference was found in other adverse reactions. Conclusion: Blonanserin has its own advantages in efficacy and safety for the treatment of schizophrenia in adults. However, given the quantity and quality of included studies, more studies are needed to confirm the former conclusion..
