[Objective] To study the post-stroke depression in elderly patients, and to screen the related influencing factors. Methods A sample of 120 elderly patients with stroke in our hospital from January 2020 to January 2022 were enrolled. All patients were assessed using 15-item Geriatric Depression Scale (GDS-15), National Institutes of Health Stroke Scale (NIHSS) and Activities of Daily Living Scale (ADL). Serological examination was also performed. The clinical characteristics, NIHSS and ADL scores of depressed and non-depressed patients were compared. NIHSS and ADL scores of patients with different degrees of depression were also compared. Then the correlation of GDS scores with NIHSS and ADL scores in post-stroke depression patients were discussed, and the independent risk factors affecting depression in elderly patients with stroke were screened by multivariate regression analysis. Results The proportion of left side of stroke lesion, and serum levels of C-reactive protein and cortisol were significantly higher in depression patients than in non-depression patients (P<0.05). Depression patients had significantly higher NIHSS score and lower ADL score than those of control group (P<0.05). Among depression patients of different severity degrees, NIHSS score was the highest in severe patients, followed by moderate patients, and was the lowest in mild patients (P<0.05), while the ADL score was the highest in mild patients, followed by moderate patients, and was the lowest in severe patients (P<0.05). Pearson correlation analysis showed that GDS score was positively correlated with NIHSS score, and negatively correlated with ADL score (P<0.05). The left side of the lesion, serum C-reactive protein, cortisol, NIHSS and ADL score were independent risk factors affecting depression in elderly stroke patients (P<0.05). Conclusion Post-stroke depression is quite common in clinical, and the severity degree of depression is closely related to the neurological deficit and activities of daily living, furthermore, left side of the lesion, serum C-reactive protein, cortisol, NIHSS and ADL score are risk factors affecting depression in elderly stroke patients.
