【摘要】探讨耐药性肺结核患者用药依从性与社会支持度、抑郁的相关性。方法 选取本院2020年1月~2023年5月收治的40例耐药性肺结核患者为耐药组,另选同期普通肺结核患者30例为普通组,同期健康者30名为对照组。评估比较三组抑郁自评量表(Self-Rating Depression Scale,SDS)、社会支持评定量表(Social Support Rating Scale,SSRS),比较耐药组与普通组简易应对方式问卷(Simplified Coping Style Questionnaire,SCSQ)及Morisky用药依从性量表8条目中文版(8-item Morisky medicationadherence scale,MMAS-8),通过Spearman秩相关分析耐药性肺结核患者MMAS-8评分与SSRS各因子评分及总分、SDS评分、SCSQ各因子评分的相关性。结果 耐药组SDS评分显著高于普通组,普通组SDS评分高于对照组,差异有统计学意义(P<0.05),耐药组主观支持、客观支持、支持利用度评分及总分显著低于普通组,普通组主观支持、客观支持、支持利用度评分及总分低于对照组,差异有统计学意义(P<0.05);耐药组MMAS-8评分、积极应对评分显著低于普通组,消极应对评分高于普通组,差异有统计学意义(P<0.05);耐药性肺结核患者MMAS-8评分与SSRS各因子评分及总分呈正相关,耐药性肺结核患者MMAS-8评分与SDS评分呈负相关,耐药性肺结核患者MMAS-8评分与积极应对评分呈正相关,与消极应对评分呈负相关,且P<0.05。结论 耐药性肺结核患者MMAS-8评分与SSRS各因子评分及总分呈正相关,耐药性肺结核患者MMAS-8评分与SDS评分呈负相关,耐药性肺结核患者MMAS-8评分与积极应对评分呈正相关,与消极应对评分呈负相关,临床医师应密切监测。
Abstract
【Abstract】Objective To explore the correlation between depression, social support, and medication adherence in drug-resistant pulmonary tuberculosis patients. Method 40 drug-resistant pulmonary tuberculosis patients admitted to our hospital from January 2020 to May 2023 were selected as the drug resistance group, 30 ordinary pulmonary tuberculosis patients in the same period were selected as the common group, and 30 healthy individuals in the same period were selected as the control group. Evaluate and compare three groups of Self Rating Depression Scale (SDS), Social Support Rating Scale (SSRS), Simplified Coping Style Questionnaire (SCSQ), and 8-item Morisky Medication Compliance Scale (MMAS-8) in Chinese, By Spearman rank correlation analysis, the correlation between MMAS-8 score and SSRS factor score, total score, SDS score, and SCSQ factor score in drug-resistant pulmonary tuberculosis patients was studied. Results The SDS score of the drug-resistant group was significantly higher than that of the ordinary group, and the SDS score of the ordinary group was higher than that of the control group, with a statistically significant difference (P<0.05). The subjective support, objective support, support utilization score, and total score of the drug-resistant group were significantly lower than those of the ordinary group. The MMAS-8 score of drug-resistant pulmonary tuberculosis patients is negatively correlated with the SDS score. The MMAS-8 score of drug-resistant pulmonary tuberculosis patients is positively correlated with the positive coping score, and negatively correlated with the negative coping score, and P<0.05. Conclusion The MMAS-8 score of drug-resistant pulmonary tuberculosis patients is positively correlated with the scores of various factors in SSRS and the total score. The MMAS-8 score of drug-resistant pulmonary tuberculosis patients is negatively correlated with the SDS score. The MMAS-8 score of drug-resistant pulmonary tuberculosis patients is positively correlated with the positive coping score, and negatively correlated with the negative coping score. Clinicians should closely monitor.
