成年抑郁患者自杀的童年期创伤、社会支持、绝望感影响研究

【摘要】探讨童年期创伤、社会支持、绝望感及抑郁程度对成年抑郁障碍患者自杀的影响。方法 连续纳入2023年4月-2023年8月在深圳市精神卫生中心93例住院患者，符合ICD-10对抑郁障碍的诊断标准的，使用一般资料问卷（包括性别、年龄、婚姻状况、文化程度等）、汉密尔顿抑郁量表（HAMD）、童年创伤问卷（CTQ）、社会支持量表（SSRS）、贝克绝望量表、简明国际神经精神访谈自杀模块、自杀行为问卷-修订版（SBQ-R）进行评估。结果 成年抑郁障碍患者的绝望感、自杀和抑郁程度两两变量间均呈正相关（P<0.01），童年创伤与自杀风险、绝望感呈正相关（P<0.01），社会支持与童年创伤、绝望感呈负相关（P<0.01），童年创伤在社会支持与绝望感中起中介作用，抑郁在绝望感和自杀中起到中介作用，绝望感在童年创伤和自杀中起中介作用。结论 绝望感和抑郁程度都增加了自杀的风险，绝望感和童年创伤、较差的社会支持有关，经历童年创伤患者对自杀的影响是通过增加绝望感而起作用。

| Abstract |
【Abstract】Objective Exploring the effects of childhood trauma, social support, despair, and degree of depression on suicide in adult patients with depressive disorder.Methods 93 patients who met the ICD-10 diagnostic criteria for depression who were hospitalized at the Shenzhen Mental Health Center from April 2023 to August 2023 were included consecutively.Evaluation was conducted using a general information questionnaire (including gender, age, marital status, education level, etc.)、 HAMD、 CTQ、 SSRS、Beck Despair Scale、MINI and SBQ-R.Results The levels of despair, suicide, and depression in adult patients with depressive disorders were positively correlated (P<0.01), childhood trauma was positively correlated with suicide risk and despair (P<0.01), and social support was negatively correlated with childhood trauma and despair (P<0.01). Childhood trauma played a mediating role in social support and despair, while depression played a mediating role in despair and suicide, Despair plays a mediating role in childhood trauma and suicide. Conclusion Despair and depression levels both increase the risk of suicide, Despair is related to childhood trauma and poor social support, and the impact of experiencing childhood trauma on suicide is mediated by increasing the sense of despair.
