【摘要】 探讨儿童青少年双相情感障碍认知功能与童年创伤的相关性。方法 选择2022年1月~2023年7月94例青少年双相情感障碍患者为观察组,另招募同期自愿参加研究的90名健康青少年为对照组。通过蒙特利尔认知评估量表(Montreal Cognitive Assessment,MoCA)评估两组认知功能,包括视空间与执行功能、命名、注意、语言、抽象、延迟回忆、定向;通过儿童期创伤问卷评估两组童年创伤,包括情感虐待、躯体虐待、性虐待、情感忽视、躯体忽视;以Pearson相关系数分析双相情感障碍青少年患者认知功能与童年创伤之间的相关性。结果 观察组MoCA总分及各项评分显著低于对照组(P<0.05)。观察组童年创伤总分及各项评分显著低于对照组(P<0.05)。经Pearson相关系数分析,青少年双相情感障碍MoCA总分及各项评分与童年创伤总分及各项评分均呈负相关(P<0.05)。结论 青少年双相情感障碍认知功能低于正常水平,童年创伤高于正常水平,且其认知功能与童年创伤呈负相关关系。
Abstract
【Abstract】Objective To explore the relationship between cognitive function and childhood trauma in children and adolescents with bipolar disorder. Methods A total of 94 adolescents with bipolar disorder from January 2022 to July 2023 were selected as the observation group, and 90 healthy adolescents who volunteered to participate in the study during the same period were recruited as the control group. The Montreal Cognitive Assessment (Montreal Cognitive Assessment, MoCA) Scale was used to assess two groups of cognitive functions, including visuospatial and executive function, naming, attention, language, abstraction, delayed recall, and orientation. The childhood trauma questionnaire was used to evaluate two groups of childhood trauma, including emotional abuse, physical abuse, sexual abuse, emotional neglect and physical neglect. Pearson correlation coefficient was used to analyze the correlation between cognitive function and childhood trauma in adolescents with bipolar disorder. Results The total score and scores of MoCA in observation group were significantly lower than those in control group (P < 0.05). The total score and scores of childhood trauma in observation group were significantly lower than those in control group (P < 0.05). Pearson correlation coefficient was used to analyze the correlation between cognitive function and childhood trauma in adolescents with bipolar disorder. Conclusions The cognitive function of adolescents with bipolar disorder is lower than normal level, and the childhood trauma is higher than normal level, and the cognitive function is negatively correlated with the childhood trauma.
