摘要
[摘要] 探讨抑郁症患者的中缝核回声与脑血管反应性的相关性。方法 选择2023年1月~2023年9月我院精神科门诊就诊的60例抑郁症病人为研究组,另选同期健康体检的70名正常人为对照组,记录并比较两组患者的临床资料、BHI、中缝核回声异常的阳性率的差异。然后分析HAMD量表评分与BHI及中缝核回声异常之间的相关性。结果 两组基本临床资料未见明显差异(P>0.05);研究组的BHI(0.58±0.09对0.84±0.10,t=-14.862,P=0.000)明显高于对照组(P<0.05);研究组中缝核回声减低的阳性率(87.5%对28.6%,x2=27.857,P=0.000)明显高于对照组(P<0.05),余指标差异比较无统计学意义(P>0.05);研究组的中缝核回声减低随HAMD量表评分升高而降低,呈负相关关系(r=-0.325;P=0.011);而BHI与HAMD量表并无线性相关(r=-0.221;P>0.05)。结论 中缝核回声减低与HAMD量表存在负相关关系,对于无法完全配合抑郁量表评估的患者,使用TCS检测中缝核回声可以作为诊断抑郁症的辅助方法
Abstract
[Abstract] Objective: To explore the correlation between the echogenicity of the raphe nuclei and cerebrovascular reactivity in patients with depression. Methods: Sixty patients with depression who visited the psychiatric outpatient clinic of our hospital from January 2023 to September 2023 were chosen as the study group, and 70 healthy individuals undergoing physical examination during the same period served as the control group. Clinical data, BHI (Breath-Holding Index), and the positive rate of echogenicity abnormalities of the raphe nuclei were recorded and compared between the two groups. The correlation between HAMD (Hamilton Depression Rating Scale) scores and both BHI and echogenicity abnormalities of the raphe nucleiwere then analyzed. Results: There were no significant differences in the basic clinical data between the two groups (P>0.05). The BHI of the study group (0.58±0.09) was significantly higher than that of the control group (0.84±0.10, t=-14.862, P=0.000, P<0.05). The positive rate of decreased echogenicity of the raphe nuclei in the study group (87.5% vs. 28.6%, x2=27.857, P=0.000) was significantly higher than that in the control group (P<0.05), with no significant differences in other indices (P>0.05). The decrease in echogenicity of the raphe nuclei in the study group was negatively correlated with the HAMD score (r=-0.325; P=0.011); however, there was no linear correlation between BHI and the HAMD score (r=-0.221; P>0.05). Conclusion: A negative correlation exists between the decreased echogenicity of the raphe nuclei and the HAMD scores. Transcranial sonography (TCS) detection of raphe nuclei echogenicity can serve as an auxiliary method for diagnosing depression in patients who are unable to fully cooperate with depression scale evaluations.
