【摘要】探讨妊娠期高血压(HIP)合并抑郁症孕产妇脑动脉血流速度变化分析。方法 本研究观察对象为2021年1月-2022年12月83例HIP孕产妇，采用爱丁堡产后抑郁量表(EPDS)评估抑郁症状，设为抑郁组以及对照组。比较两组孕产妇一般资料，经颅多普勒超声检测搏动指数(PI)、平均血流速度(Vm)、阻力指数(RI)等大脑中动脉血流动力学参数，分析上述参数与抑郁程度的相关性。结果 83例HIP孕产妇检出抑郁24例，检出率28.92%;;抑郁组产次、文化程度差异具有统计学意义（P<0.05），两组年龄、孕周、居住地差异无统计学意义（P>0.05）；抑郁组Vm显著高于对照组，PI、RI显著低于对照组，（P<0.05）；EPDS评分与Vm呈正相关，与PI、RI呈负相关（P<0.05）。结论 HIP合并抑郁症孕产妇存在大脑中动脉血流动力学异常情况，表现为Vm升高，PI、RI降低，且与抑郁程度有关。

| Abstract |
[Abstract] Objective To investigate the analysis of maternal cerebral artery blood flow velocity changes in hypertension in pregnancy (HIP) combined with depression. Methods In this study, 83 maternal cases of HIP were observed from January 2021 to December 2022, and depressive symptoms were assessed using the Edinburgh Postpartum Depression Scale (EPDS) and were set up as a depression group as well as a control group. The general data of the two groups were compared, and the hemodynamic parameters of the middle cerebral artery such as pulsatility index (PI), mean blood velocity (Vm) and resistance index (RI) were measured by transcranial Doppler ultrasound. The correlation between the above parameters and the degree of depression was analyzed. Results Depression was detected in 24 cases of 83 HIP pregnant women, with a detection rate of 28.92%. The differences in the number of births and literacy of the depressed group were statistically significant (P<0.05). The differences in age, gestational week, and place of residence between the two groups were not statistically significant (P>0.05). Vm was significantly higher and PI and RI were significantly lower in the depressed group than in the control group (P<0.05). EPDS scores were positively correlated with Vm and negatively correlated with PI and RI (P<0.05). Conclusion Maternal HIP combined with depression has abnormal middle cerebral artery hemodynamics, as evidenced by elevated Vm and decreased PI and RI, and is associated with the degree of depression.
