| 摘要 |
分析社区健康管理与社区护理在老年高血压（HBP）及糖尿病（DM）患者中的应用效果。方法 纳入研究的患者为老年HBP及DM患者78例，随机分为观察组（社区健康管理与社区护理）和对照组（基础护理）各39例，对比效果。结果 观察组血糖、血压低于对照组（P<0.05）；观察组自我管理能力高于对照组（P<0.05）。结论 老年人HBP及DM患者采取社区健康管理和社区护理对于血糖、血压的控制十分有利，效果满意。

| Abstract |
Objective To analyze the effect of community health management and community nursing on elderly patients with hypertension (HBP) and diabetes mellitus (DM). Methods 78 elderly patients with HBP and DM were randomly divided into observation group (community health management and community nursing) and control group (basic nursing), 39 cases each, to compare the effect. Results Blood glucose and blood pressure in observation group were lower than those in control group (P<0.05). The self-management ability of observation group was higher than that of control group (P<0.05). Conclusion Community health management and community nursing in elderly patients with HBP and DM are very beneficial to the control of blood sugar and blood pressure, and the effect is satisfactory.
