Objective To analyze the study of health education and chronic disease management and nursing in elderly people with hypertension in community. Methods 35 elderly patients with hypertension admitted to our hospital from January 2023 to January 2024 were selected as the study objects, and they were randomly divided into control group (17 cases, receiving routine nursing management) and observation group (18 cases, receiving health education and chronic disease management and nursing). The scores of anxiety degree, depression degree, diastolic blood pressure, systolic blood pressure, disease cognition level and nursing satisfaction of the two groups were compared and evaluated. Results The diastolic blood pressure, systolic blood pressure, anxiety degree score and depression degree score of observation group were lower than those of control group (P < 0.05). Blood pressure normal range, related complications, risk factors, healthy lifestyle and other disease cognition scores in the observation group were higher than those in the control group (P < 0.05). The nursing satisfaction of observation group was higher than that of control group (P < 0.05). Conclusion The implementation of health education and chronic disease management and nursing for elderly hypertension patients in communities can achieve obvious therapeutic effects, significantly improve patients negative emotions, reduce blood pressure, and improve nursing satisfaction, which is worthy of promotion and application.
