职业倦怠护士大脑形态学结构改变的初步探索

摘要
探究职业倦怠护士是否存在大脑形态学结构的异常改变,并分析其与职业倦怠量表评分的相关性。方法 选取30名职业倦怠护士和29名无职业倦怠护士行静息状态下磁共振成像(magnetic resonance imaging,MRI)扫描,使用SPM12工具箱中的CAT12软件对数据进行预处理,通过DPABI平台的statistical analysis对两组被试的大脑灰质体积参数图进行统计分析。结果 与对照组相比,职业倦怠组护士灰质体积增大的脑区为左侧眶部额上回、左侧眶部额中回和右侧额中回;灰质体积减小的脑区为右侧小脑1 区、2 区和8 区。(GRF矫正,单个体素 P <0.001,校正后 P <0.05)。相关性分析显示,灰质体积改变脑区与护士职业倦怠量表各维度评分之间均无显著相关性(P>0.05)。结论 职业倦怠护士部分脑区灰质体积发生改变,提示职业倦怠护士可能存在大脑皮层功能区的萎缩或增生。
Abstract
Objective Exploring the changes in brain morphology of nurses in burnout group and control group to analyze the correlation between different brain areas and burnout scale scores, and explore job burnout whether the nurse has abnormal changes in brain morphology. Methods 30 burnout nurses and 29 burnout-free nurses with matching age, nursing age, gender and education years were selected for resting magnetic resonance imaging (MRI) scans. Results Compared with the control group, the brain areas with increased gray matter volume in the burnout group included the left upper orbital frontal gyrus, the left orbital middle frontal gyrus, and the right middle frontal gyrus; the brain areas with reduced gray matter volume were on the right Cerebellum 1, 2, and 8 areas. (GRF correction, single voxel P<0.001, after correction P<0.05). The results of correlation analysis showed that there was no significant correlation between the changes in gray matter volume and the scores of each dimension of the Maslach Burnout Inventory(MBI) (P>0.05). Conclusion The gray matter volume of some brain areas of burnout nurses changed, suggesting that burnout nurses may have atrophy or hyperplasia of cerebral cortex function area.
