摘要 目的:分析重度颅脑损伤患者术后应用低频重复经颅磁刺激联合奥拉西坦对认知功能、脑血流及不良情绪的影响。方法:选取2019年12月至2021年12月我院收治的91例重度颅脑损伤术后患者为研究对象,采用抽签的方法将两组患者分组。对照组45例给予奥拉西坦治疗,观察组46例增加低频重复经颅磁刺激。12周后,对比两组患者认知功能、脑血流、不良情绪。结果:观察组患者顺背测试、简易精神状态检查量表(MMSE)评分均高于对照组(P<0.05);观察组患者大脑中动脉平均血流速度(Vm)、收缩期峰流速(Vs)高于对照组,舒张期末血流速度(Vd)、搏动指数(PI)低于对照组(P<0.05);观察组患者汉密尔顿抑郁量表(HAMD)评分、汉密尔顿焦虑量表(HAMA)评分均低于对照组(P<0.05)。结论:低频重复经颅磁刺激联合奥拉西坦能够有效减轻重度颅脑损伤术后患者不良情绪,改善认知功能及脑血流循环。
Abstract
Abstract Objective: To analyze the effects of low frequency repetitive transcranial magnetic stimulation (RTMS) combined with olacetam on cognitive function, cerebral blood flow and adverse mood in patients with severe craniocerebral injury. Methods: A total of 91 patients with severe craniocerebral injury after surgery in our hospital from December 2019 to December 2021 were selected as the research object, and the two groups of patients were divided into two groups by drawing lots. The control group was treated with oracetam in 45 cases, and the observation group was treated with low frequency repeated TRANSCranial magnetic stimulation in 46 cases. After 12 weeks, cognitive function, cerebral blood flow and bad mood were compared between the two groups. Results: The scores of anterior-back test and MMSE in observation group were higher than those in control group (P<0.05). The mean blood flow velocity (Vm) and peak systolic flow velocity (Vs) of middle cerebral artery in observation group were higher than those in control group, and the diastolic blood flow velocity (Vd) and pulsatile index (PI) in observation group were lower than those in control group (P<0.05). The scores of Hamilton Depression Scale (HAMD) and Hamilton Anxiety Scale (HAMA) in observation group were lower than those in control group (P<0.05). Conclusion: Low frequency repetitive transcranial magnetic stimulation combined with olacetam can effectively relieve adverse mood, improve cognitive function and cerebral blood flow circulation in patients with severe craniocerebral injury after surgery.
