述情障碍对慢性精神分裂症患者肥胖的影响

摘要
探究慢性精神分裂症患者述情障碍与其肥胖的相关性,以期为慢性精分患者肥胖的预防和治疗该群体防控提供更多理论依据和方法措施。方法 纳入住院精神分裂症患者485例,其中肥胖组172例,正常组BMI正常组313例。用标准化方法测量身高、体重,计算BMI来描述患者的肥胖程度。采用阳性与阴性症状量表(PANSS)评估患者的精神病性症状,多伦多述情障碍量表(TAS-20)评估患者有无述情障碍。结果 述情障碍发生率在两组分别为32.6%(肥胖组)和26.5%(BMI正常组),具有统计学差异(P<0.05)。精神分裂症患者BMI指数与TAS-20总分(r=0.121,P<0.01)、情感辨别不能(r=0.116,P<0.05)、情感描述不能(r=0.147,P<0.01)呈显著正相关 (P<0.05)。结论 慢性精神分裂症伴肥胖患者述情障碍发生率高于BMI正常组,肥胖和述情障碍相关,尤其在情感辨别和情感描述能力方面显著相关。
Abstract
Objective To explore the correlation between alexithymia and obesity in chronic schizophrenic patients, in order to provide more theoretical basis for the prevention and control of this group. Methods 485 inpatients with schizophrenia were included, including 172 cases in obese group and 313 cases in normal BMI group. Height and weight were measured by standardized methods, and BMI was calculated to describe the degree of obesity. The Positive And Negative Symptom Scale (PANSS) was used to evaluate the psychotic symptoms of the patients, and the Toronto Alexithymia Scale (TAS-20) was used to evaluate whether the patients had alexithymia. Results The incidence of alexithymia in the two groups were 32.6% (obese group) and 26.5% (normal BMI group), respectively (P <0.05). BMI index of schizophrenic patients was significantly positively correlated with the total score of TAS-20 (r = 0.121, P < 0.01), inability to distinguish emotion (r = 0.116, P < 0.05) and inability to describe emotion (r = 0.147, P < 0.01) (P < 0.05). Conclusion The incidence of alexithymia in patients with chronic schizophrenia and obesity is higher than that in normal BMI group. Obesity is related to alexithymia, especially in emotional discrimination and emotional description.
