基于rs-fMRI比率低频振幅及动态功能连接对强迫症脑功能的初步研究

摘要
采用静息态功能磁共振成像 (rs-fMRI) 方法,探讨静息状态下强迫症患者局部脑功能及全脑动态功能连接模式。方法 静息状态下采集强迫症患者 (n=50) 及健康对照者 (n=50) 脑功能数据;通过比率低频振幅 (fractional amplitude of low-frequency fluctuations, fALFF) 算法探讨强迫症患者局部脑自发神经活动,采用动态功能连接 (dynamic functional connectivity, dFC) 分析方法计算强迫症患者静息状态下全脑动态功能连接模式;采用偏相关分析方法分析异常的fALFF值和dFC值与临床症状之间的相关性;采用支持向量机 (support vector machines, SVM) 的方法探讨异常的局部脑功能和动态功能连接是否可以用于强迫症的识别。结果 与健康对照者相比,强迫症患者组静息状态下右侧额中回fALFF值升高 (t=3.6967; P<0.05, 高斯随机场理论 (Gaussian random field theory, GRF) 校正);左侧颞上回fALFF值降低(t=-3.9716; P<0.05, GRF校正);右侧额中回与左侧舌回、左侧楔前叶、右侧距状裂dFC值降低 (t=-3.6705, -4.0795, -3.9618; P<0.05, GRF校正);异常的fALFF值和dFC值与临床症状之间均无显著相关性;SVM结果表明fALFF值和dFC值异常的脑区不能很好的区分强迫症患者和健康对照。结论 静息状态下,强迫症患者的右侧额中回、左侧颞上回存在局部自发神经活动和全脑动态功能连接的异常。
Abstract
Objective The resting-state functional magnetic resonance imaging (rs-fMRI) method was used to explore the local brain function and dynamic functionalconnection mode of patients with obsessive-compulsive disorder at rest. Methods Functional brain data was collected from OCD patients (n=50) and healthy controls (n=50) in rest. The fractional amplitude of low-frequency fluctuations (fALFF) approach was used to explore local spontaneous neural activities, dFC analysis was used to investigate the whole-brain dFC patterns at rest in patients with OCD. Partial correlation analysis was used to analyze the relationship between abnormal fALFF and dFC values and the clinical symptoms in patients with OCD. The support vector machines (SVM) was used to explore whether brain regions with abnormal function and dynamic connectivity could be used for the recognition of OCD. Results Compared with controls group, patients with OCD showed increased fALFF value in the right middle frontal gyrus (t=3.6967; P<0.05, Gaussian random field theory (GRF) corrected) and reduced fALFF value in the left superior temporal gyrus (t=-3.9716; P<0.05, GRF corrected); reduced dFC between the right middle frontal gyrus and the left lingual gyrus, and the left precuneus, and the right calcarine (t=-3.6705, -4.0795, -3.9618; P<0.01, GRF corrected). No significant correlation between abnormal fALFF and dFC values and clinical symptoms. SVM results showed that brain areas with abnormal fALFF and dFC values were not well differentiated between OCD patients and healthy controls. Conclusion The left superior temporal gyrus and the right middle frontal gyrus showed abnormal local spontaneous neural activities and whole-brain dFC at rest in patients with OCD.
