【摘要】探讨皮质下血管性认知功能障碍(SIVD-CI)患者脑萎缩磁共振(MRI)特点。方法 本研究观察对象为2020年1月-2022年6月76例皮质下缺血性血管病(SIVD )患者,评估简易智能精神状态量表 (MMSE)并设为SIVD-CI组(41例)以及对照组(35例)。进行3.0T头颅MRI扫描,采用视觉等级评分评价额叶、颞叶、顶叶萎缩情况以及脑白质疏松情况,并分析SIVD-CI组患者MMSE评分与之的相关性。结果 SIVD-CI组额叶、颞叶、顶叶以及脑萎缩总分显著高于对照组,(P<0.05);SIVD-CI组额叶、颞叶、顶叶以及脑白质疏松总分显著高于对照组,(P<0.05);SIVD-CI组患者MMSE评分与脑萎缩总分、脑白质疏松总分呈负相关(P<0.05)。结论 SIVD-CI患者MRI提示额叶、颞叶、顶叶区存在脑萎缩以及脑白质疏松等情况,临床应给予重视。
Abstract
[Abstract] Objective To investigate the magnetic resonance (MRI) characteristics of brain atrophy in patients with subcortical vascular cognitive dysfunction (SIVD-CI). Methods The study was observed in 76 patients with subcortical ischemic vascular disease (SIVD) from January 2020 to June 2022. The Simple Mental State of Intelligence Scale (MMSE) was assessed and set up as SIVD-CI group (41 patients) as well as control group (35 patients). 3.0T cranial MRI scan was performed. The visual grade score was used to evaluate frontal, temporal and parietal atrophy and cerebral white matter sparing. The correlation between the MMSE scores and the patients in the SIVD-CI group was also analyzed. Results The frontal, temporal, parietal and total cerebral atrophy scores were significantly higher in the SIVD-CI group than in the control group (P<0.05). the frontal, temporal, parietal and total cerebral white matter atrophy scores were significantly higher in the SIVD-CI group than in the control group (P<0.05). the MMSE scores were negatively correlated with the total cerebral atrophy and total cerebral white matter atrophy scores in the SIVD-CI group (P<0.05). Conclusion MRI of patients with SIVD-CI suggests the presence of brain atrophy in the frontal, temporal, and parietal regions as well as cerebral white matter sparing, which should be given clinical attention.
