Objective To study the effect of self-management in patients with very low platelet using nurse-led case management model. Methods This study on self-management of patients with extremely low platelets was conducted in our department from January 2022 to January 2023. 76 patients with extremely low platelets were systematically divided into the case management group and the management control group, with 38 cases in each group. The self-management effects and patient satisfaction of patients with extremely low platelets were compared between the two groups. Results In the control group, 30 patients had good self-management effect, accounting for 78.95%; in the case management group, 36 patients had good self-management effect, accounting for 94.74%; there was a significant difference in the data of self-management effect between the two groups of patients with very low platelet content (P<0.05). There were 32 patients in the case management group with higher satisfaction, accounting for 84.21%; compared with the management control group, there were 24 patients with higher satisfaction, accounting for 63.16%; there was a significant difference in satisfaction between the two groups (P<0.05). Conclusion Self-management of patients with very low platelet is extremely important. Although the routine care of patients with very low platelet will inform them of relevant self-care precautions through health education, adverse events often occur due to the specificity of the disease, so nurse-led case management model is adopted. It can make patients have a deeper sense of involvement in self-disease management, and better results in self-management effectiveness and emotional state.
