| 摘要 |
探究慢性心力衰竭患者应用个案全程管理模式护理的效果。方法 选择本院2021.04-2022.02收治慢性心力衰竭病人共计70名，以入院时间线法平均分出两组，即为常规组（2021.04-2021.08）、实验组（2021.09-2022.02），每组35名设为研究对象。常规组予以常规管理模式护理、实验组予以个案全程管理模式护理，对比两组患者自我管理评分、生活质量评分。结果 相较常规组，实验组患者自我管理评分更高，P＜0.05。相较常规组，实验组生活质量评分更高，P＜0.05。结论 慢性心力衰竭患者应用个案全程管理模式护理可提高其生活质量，强化自我管理能力获得理想预后结果，建议普及应用。

| Abstract |
Objective To explore the effect of case whole process management nursing on patients with chronic heart failure. Methods a total of 70 patients with chronic heart failure admitted to our hospital from April 2021 to February 2022 were selected. They were divided into two groups, namely, the routine group (April 2021 to August 2021) and the experimental group (September 2021 to February 2022), with 35 subjects in each group. The routine group was given routine management mode nursing and the experimental group was given case whole process management mode nursing. The Self-management score and quality of life score were compared between the two groups. Results compared with the routine group, the self-management score of the experimental group was higher (P<0.05). Compared with the routine group, the score of quality of life in the experimental group was higher (P<0.05). Conclusion the application of case whole process management model nursing in patients with chronic heart failure can improve their quality of life, strengthen their self-management ability and obtain ideal prognosis. It is recommended to popularize the application.
