Objective With the improvement of peoples economic level, great changes have taken place in diet and life. Unhealthy lifestyles are very harmful to peoples health. Chronic diseases are also rapidly invading peoples bodies. Therefore, this paper analyzes The application effect of health education in chronic disease management, and its practical application value. Methods A retrospective analysis was made of 150 chronic disease patients received by our hospital in two communities in January 2021. According to the two communities, they were divided into two groups, the control group and the observation group (75 cases in each group). The two groups were compared. Patients perceptions of the disease and their own health behaviors. Results Statistical analysis showed that after the intervention, the two groups of patients were compared and analyzed on the awareness rate of disease knowledge and healthy behaviors. The scores of the research group were all significantly higher, and the data differences between the groups were significant P<0.05, showing the experimental value. Conclusion Health education plays an important role in the management of chronic diseases. The implementation of health education enables patients to have a better understanding of their own disease-related knowledge, improves the knowledge awareness rate of patients, and then enhances the awareness of chronic disease prevention and treatment, and improves their own health behavior.
