| 摘要 |
探讨一体化健康管理和长期护理在住院老年衰弱患者中的应用效果。方法 选取2023年5月到2024年5月期间我院收治的住院老年衰弱患者共60例随机分为对照组与观察组，每组30例，对照组采用常规护理，观察组采用一体化健康管理和长期护理。对比两组日常生活活动能力、健康行为、自我护理能力以及生活质量。结果 观察组生活活动能力、健康行为、心理状态以及生活质量均优于对照组（P＜0.05）。结论 一体化健康管理和长期护理能有效提升住院老年衰弱患者的健康水平和生活自理能力，值得在临床推广应用。

| Abstract |
Objective To explore the application effect of integrated health management and long-term care in hospitalized elderly patients with frailty. Methods A total of 60 hospitalized elderly patients with frailty admitted to our hospital from May 2023 to May 2024 were randomly divided into a control group and an observation group, with 30 cases in each group. The control group received routine care, while the observation group received integrated health management and long-term care. Compare the daily living activity ability, health behavior, self-care ability, and quality of life between two groups. Results The observation group had better life activity ability, healthy behavior, psychological state, and quality of life than the control group (P<0.05). Conclusion Integrated health management and long-term care can effectively improve the health level and self-care ability of hospitalized elderly patients with frailty, and are worthy of clinical promotion and application.
