| 摘要 |
探讨社区老年高血压采取健康教育、慢性病管理护理的效果。方法 选取2022年8月至2024年9月本社区老年高血压患者76例，随机分为观察组(健康教育、慢性病管理护理)和对照组(常规护理)各38例，对比效果。结果 观察组不良情绪评分、血压水平低于对照组，观察组自我管理行为评分高于对照组(均P<0.05)。结论 健康教育联合慢性病管理护理应用于社区老年高血压患者中效果良好。

| Abstract |
Objective To explore the effect of health education and chronic disease management nursing on elderly patients with hypertension in the community. Methods A total of 76 elderly patients with hypertension in the community from August 2022 to September 2024 were selected and randomly divided into an observation group (health education, chronic disease management nursing) and a control group (conventional nursing), 38 cases in each group, to compare the effects. Results The negative emotion score and blood pressure level in the observation group were lower than those in the control group, and the self-management behavior score in the observation group was higher than that in the control group (all P<0.05). Conclusion Health education combined with chronic disease management nursing has a good effect on elderly patients with hypertension in the community.
